Financial and Billing Policy

We’re ready to work with you to help you get the health care you need, regardless of your ability to pay. This section explains how we manage financial and insurance business.

Primary and Secondary Insurance Information

We’ll need to know complete, accurate, and up-to-date information about whatever health insurance policies you may have, as well as your commitment to pay for the services our office will provide. If you don’t have health insurance, we may be able to help you enroll in a subsidized policy or qualify you to enroll in our Sliding Fee Scale program.

Sliding Fee Scale Information

Our Sliding Fee Scale Program allows patients who are uninsured or underinsured to receive healthcare services at a reduced cost. To be eligible for a Sliding Fee Discount, one must provide evidence of household income not exceeding double the federal poverty level, which varies depending upon the size of the family and the number of people that live in the household. The maximum Sliding Fee Discount available is 100% and the minimum payment required from self-pay patients to receive service is $10.
